Presbyopia correction with femtosecond laser-assisted cataract surgery: the mix and match principle

Purpose:
To study the value of mixing and matching presbyopia correcting multifocal posterior chamber intraocular lenses to induce modified monovision for improving near, intermediate, and distant vision in patients having femtosecond laser assisted cataract surgery (FLACS).
Setting:
Cataract and Glaucoma Unit, Magrabi Eye Center, Jeddah, Saudi Arabia
Methods:
Patients with bilateral visually significant cataract who are willing to do FLACS with presbyopia correction were counseled for mix-and-match of multifocal ReStor intraocular lenses +3.0D (Alcon) in one eye and +2.5D in the other eye. All patients were interviewed for their visual needs and expectations after surgery and tested for accepting the idea of modified monovision. All first operated eyes had ReStor +3.0D and all second operated eyes had ReStor +2.5D lenses. Monocular and binocular uncorrected near, intermediate, and distant visual acuities were tested and patients’ satisfaction was evaluated in various settings of his ordinary visual activities.
Results:
Twenty-four eyes of 12 patients were included in the study, 5 males and 7 females. All eyes had uneventful FLACS. Femtolaser astigmatic keratotomy was done in 4 eyes for astigmatism greater than 0.5D and less than 1.25D. Uncorrected binocular distance visual acuity was 20/20 in all eyes. Mean bilateral uncorrected near vision was J 1.20 and intermediate vision was J 2.15. All patients had complete spectacle independence during their ordinary life activities such as reading, computer work, and driving. Two patients complained of mild halos and glare that did not affect his daily activities and required assurance without any further intervention. Results were comparable to those of mix-and-match of other multifocal and accommodating IOLs in the literature.
Conclusions:
Mix-and-match of multifocal ReStor intraocular lenses with different add powers achieved a good patient’s visual satisfaction at near, intermediate, and distant daily visual activities.
