Evaluation of macular density, pigment density, contrast sensibility and chromatic vision after SA60 Alcon intraocular lens (IOL) implantation

Purpose:
: The natural lens protects the retina against retinal pathology ultraviolet-induced. I order, to reduce the risk, the use of ultraviolet light-absorbing IOL has become the goldstandard. However, it has been reported that the implantation of clear IOLs increases the risk for retinal pathology by allowing blue light to reach the retina.
The purpose of this study is to evaluate the macular pigment density, contrast sensibility and chromatic vision before and after cataract surgery and SA 60 Alcon IOL implantation
Setting:
Ophthalmology department of Garcia de Orta Hoapital, Almada, Portugal
Methods:
: 26 eyes of 13 patients were evaluated 1 week before surgery, 1 month and 3 months after surgery considering the best corrected visual acuity (BCVA), biomicroscopy, fundoscopy, chromatic vision using Farnsworth-Munsell 100-Hue and contrast sensibility with CSV 1000E test.
The macular pigment density was measured through QuntifEye Macular Pigment Screener based on the Heterochromatic flicker photometry principle.
Eyes with retinal, ocular inflammatory and corneal diseases were excluded, such as patients with history of ocular surgery, with medication which can interfere with macular pigment density (e.G.plaquinol) and with BCVA <5/10 in Snellen scale.
Results:
The BCVA before cataract surgery was 0,55±0,065 and 0,98±0,064 through Snellen scale. The mean macular pigment density befre, 1 month and 3 months after cataract surgery was 0,458±0,145, 0,462±0,141, 0,462±0,141, respectively. There were no statistical significantdifefrence between MPD before and after cataract surgery (p>0,05).
There were a higher percentageof patients with a rise in the contrast sensibility for every spatial frequency of CSV 1000E test with 40,% of patients answering 6 for 3 cycles/degree, 44,4% answering 5 for 6 cycles/degree, 55,5% 5 for 12 cycles/degree and 48,1% 6 for 18 cycles/degree 3 months after surgery.
Conclusions:
We conclude that the removal of natural lens and implantationof a clear SA 60 Alcon IOL doesn’t interfere in MPD through time. There is a significant rise in contrast sensibility in this type of surgery. This rise of contrast sensibility occurs essentially in the first month after surgery.
The implantation of a clear IOL maintains visual acuity and doesn’t rise the risk of retinal pathology due to blue light.
