Optical quality of a diffractive intraocular lens with extended range of vision

Abstract Details

Purpose:

To objectively assess the optical quality of the new TECNIS® Symfony (Abbott Medical Optics Inc., Santa Ana, CA) model ZXR00, a new diffractive pseudophakic intraocular lens (IOL) with an extended range of vision.

Setting:

University of Valencia, Spain.

Methods:

The image quality of the Tecnis Symfony IOL was assessed by means of the PMTF optical bench (LAMBDA-X, Nivelles, Belgium). The device obtains measurements of the modulation transfer function (MTF) as through-focus and through-frequency curves. It complies with International Organization for Standardization (ISO) 11979-2 and 11979-9 requirements for the characterization of multifocal IOLs. The measurements were taken at the centered position of the lens, as well as at decentered positions by 0.2 and 0.4 mm. Different pupil sizes were set in order to assess the behaviour of the IOL at photopic (3.0 mm) and mesopic conditions (4.5 mm).

Results:

The assessment of the IOL showed that the continuous range is observed in the through-focus figure analysis as wide areas in which the covered distances are located around two MTF peak values. The values of the MTF were decreased at 4.5 mm pupil size compared to the 3.0 mm aperture. For the considered focal planes, decentration did not markedly affect the MTF values. The differences in MTF with decentration where lower than 0.05 when we considered a spatial frequency of 50 cycles/mm, which would approximately correspond to a 0.8 decimal visual acuity.

Conclusions:

The optic quality of IOLs described in terms of the MTF should be expected to have an impact on the visual quality of patients. It could be suggested that under photopic conditions, the TECNIS Symfony IOL would be related to better visual quality, as well as for bright indoor illuminated activities. Decentering up to 0.4 mm did not significantly alter the optical quality of the lens. However, further studies should be carried out in order to evaluate the coupling effect of different corneal profiles with this IOL, with the purpose of obtaining more specific clinical conclusions.
