Comparative analysis of phakic IOL implantation results

Abstract Details

Purpose:

. Excimer laser ablation for refractive purpose is impossible in high degree myopia provided the cornea is "thin". An alternative method of refraction abnormality correction in such cases is implantation of phakic intraocular lenses (PIOLs). For the same purpose, we use IOL Cachet (Alcon) (group I), an anterior chamber lens with four fixation points in ocular anterior chamber angle, and Artiflex (Ophtec) (group II) with optic part made of polysiloxane and rigid haptic part fastened by iris. The purpose is to study and compare the functional and anatomic results of implantation of various anterior chamber PIOL models.

Setting:

Ulyanovsk Regional Clinical Hospital, Russia

Methods:

8 cases of implantation of each PIOL were selected for the study. Mean baseline refraction in group I was (-)15, 25D. Mean subjects' age was 23.5 years old. Corrected visual acuity was 0.6 to 1.0 (0.8). The mean corneal endothelium density was 2860 cells per mm.2 Mean baseline refraction in group II was (-) 16.5 D. Mean subject age was 26 years old. Visual acuity was 0.3 to 1.0 (mean 0.7) with the highest tolerable correction. The mean endothelium density in this group was 2800 cells per mm.2 All the operations were unremarkable.

Results:

Follow-up period was 6 months to 2.5 years. Mean uncorrected visual acuity in group I was 0.9 - in 1 month and 0.8 - in 1 year after the surgery. Mean uncorrected visual acuity in group II was 0.7 - in 1 month and 0.8 - in 1 year. The mean endothelium density in group I was 2790 (-2.3%) in 1 year after the surgery. Mean endothelial cell loss within 1 year after the surgery was 1.9% (2660 to 3300). Residual ametropy in group I was -0.25D on average, and in group II -/+0.5 D on average.

Conclusions:

Both groups with comparative preoperative eye parameters showed high values of uncorrected visual acuity exceeding maximal baseline corrected visual acuity following implantation of various models of anterior chamber PIOLs. Accommodation maintenance assures high vision quality and independence of correction means. Integrity of corneal endothelium during follow-up period allows considering implantation of the above PIOL models as safe. Implantation of phakic lenses of these models is a highly effective, predictable and safe intervention in high-degree myopia in special cases non-suitable for excimer laser correction.
