Association of pseudoexfoliative syndrome with increased vascular risk: myth or reality?

Abstract Details

Purpose:

To analyse whether patients with pseudoexfoliative (PEX) syndrome have increased vascular risk compared with those without the syndrome. To register and compare the distribution of vascular risk factors and to calculate and compare the individual vascular risk in patients with and without PEX syndrome.

Setting:

Ophthalmology department, Hospital Universitario Donostia. San Sebastian (Spain)

Methods:

90 patients with PEX syndrome were compared with 250 patients without PEX syndrome, distributed according sex and age. History of ischemic heart disease and vascular risk factors (smoking, diabetes mellitus, arterial blood pressure and levels of total cholesterol, HDL and LDL) were registered by the medical history of each patient. Data from the last blood analysis and the average of last 3 measurements of blood pressure were used. Individual vascular risk was calculated using SCORE (Systematic Coronary Risk Evaluation), REGICOR (Registre GironĂ­ del cor) and ERICE (Native Cardiovascular Score for the Low-risk and Aged Mediterranean Population of Spain) scales.

Results:

Presence of ischemic heart disease in PEX group is 9,5% and in non-PEX group is 10,3% (p=0,2). The vascular risk by Regicor score is 5,2% in PEX group and 5,6% in non-PEX group. According to SCORE scale, the vascular risk in PEX group is 3,95% and 3,7% in non-PEX group. Applying ERICE scale, the vascular risk in PEX group is 36,9%, while the risk in non-PEX group is 34,8%. There are no differences in both groups in regard to the distribution of vascular risk factors.

Conclusions:

Our study suggests that there is no association between exfoliative syndrome and the presence of ischaemic heart disease. We also found no differences in the vascular risk of patients with and without PEX syndrome.
