Purpose:
The authors present a case-report of a patient with a high voltage electrical burn with cataract formation.
Setting:
Centro Hospitalar Lisboa Central, Lisbon, Portugal
Methods:
A 59 years old male patient was referred to the Ophthalmology outpatient clinic with presbyopia complaints. He had suffered a high-voltage electrical burn 15 years earlier. At that time no visual symptoms were present and examination was normal. No early onset cataract formation was observed. A careful ophthalmological follow-up examination with fundoscopy and IOP measurement was performed afterwards. Visual field examination with static computerized perimetry (Octopus®), anterior segment OCT (Visante®) and posterior segment OCT were performed. Specular microscopy with endothelial cell count was also obtained.
Results:
The patient presented bilateral cataracts with stellate anterior subcapsular opacities more advanced in the left eye that were photographed. Fundoscopy didn’t reveal any abnormalities. IOP was 14mmHg bilaterally. Visual Acuity was 1.0 (Decimal) in both eyes. Visual field examination, retinography and OCT scans were normal. Endothelial cell count was normal when age-adjusted.
Conclusions:
The patient only presented lenticular opacities as a late onset consequence of the electrical burn. Visual acuity wasn’t significantly affected by the cataract and further extensive ophthalmological examination failed to discover further injuries such as optic nerve and retina damage. A complete visual rehabilitation was secured. Further follow-up for senile cataract diagnosis should be performed.
