Corneal epithelial thickness mapping by optical coherence tomography in normal eyes and corneal diseases

Abstract Details

Purpose:

To map the corneal epithelial thickness in vivo with Fourier-Domain Optical Coherence Tomography in normal eyes and corneal pathologies.

Setting:

The study was done at Universitary and Politechnic Hospital La Fe.

Methods:

This is a cross-sectional observational study. Corneal epithelium over the entire cornea was topographically imaged using a novel Optical Coherence Tomography system (RTVue-100, Optovue Inc). An epithelial thickness map was automatically generated. Epithelial thicknesses of the central 2-mm, paracentral 2- to 5-mm, and midperipheral 5- to 6-mm zones were obtained.

Results:

The average epithelial thickness of the central, paracentral and midperipheral zones was 56.4 ± 1.0 μm, 52.2 ± 3.2 μm, and 57.3 ± 2.0 μm, respectively, in normal eyes. Compared with normal control subjects, eyes with corneal diseases (keratoconic, postherpetic leucoma, endothelial Fuchs dystrophy, intraestromal rings, adenoviral infiltrates…) had different thinner epithelial thickness in all three zones.

Conclusions:

We should take note of the nonuniformity of the paracentral and midperipheral corneal epithelium thicknesses. The study of anatomical changes through analysis of Fourier-Domain Optical Coherence Tomography corneal epithelial maps is helpful in the management and monitoring of patients with corneal diseases. It provides important additional information and is a good complement to the anatomical study and provides a novel benchmark for future and comparative studies.
