Abstract Details
Purpose:
Uncovering ocular surface by dermoid excision followed by covering with lamellar graft is effective modality for good visual rehabilitation and ocular surface reconstruction especially in large dermoid. This video presents the management of dermoid grade 2 with novel technique of dermoid excision with lamellar keratoplasty.

Methods:
A 8 year old boy having large dermoid OD with visual acuity (VA) of 1/60 was undergone dermoid excision with lamellar keratoplasty. Our case belonged to grade II with corneal involvement sparing deep stroma, descemet's membrane, and endothelium which was elucidated well on UBM. Lesion is excised to its entirety and a lamellar graft is tailored to fit the defect of both cornea and sclera. Donor corneal button 6.5 mm sized, stripped of descemet's membrane was placed onto the bare descemet's membrane and sclera. Graft was attached with the help of glue and few sutures.
Results:
Post operative VA was 6/60. Other eye partial occlusion has been started from 1st postop day. Suture removal and refraction was planned after 4 weeks.
Conclusions:
Simple excision is generally not sufficient by itself to manage an extensive lesion. Dermoid excision with lamellar keratoplasty is effective modality in the management of large dermoids.
