Standardising postoperative Descemet's stripping endothelial keratoplasty (DSEK) care

Abstract Details

Purpose:

At present, there remains no published guidance for evaluating surgical outcomes post-operatively following a DSEK operation. This study aims to investigate the various post-op measurements undertaken by doctors in clinics following a DSEK operation and to standardize post-operative DSEK measurements.

Setting:

Clinics at Ashford and St Peters hospital NHS foundation trust.

Methods:

Patients who had DSEK operation from the period of May 2012 to June 2014 had their notes retrospectively analysed. Post-operative examination of these patients which includes measurements such as endothelial cell count pre and post operatively, refraction status post surgery and pre and post op central corneal thickness were measured.

Results:

There were 25 patients who had DSEK operation following this period. The mean age was 75±8, of this, 72% were female and 28% were male. From this sample, 19 had DSEK for Fuchs corneal drystrophy, 5 for psuedophakic bullous keratopathy and 1 for ididocorneal endothelial syndrome. Only 68% of patients had their post-operative endothelial cell counted, and only 72% had refraction post-operatively. In addition to this, only 48% of patients had their central corneal thickness measured pre and post-operatively. All the patients however had the pre and post-operative visual acuities measured.

Conclusions:

The above results show that measurements at the post-operative assessment are very variable and depend greatly on the surgeon conducting the post operative follow up. Hence, there is a need for standardized proforma to measure surgical outcomes following corneal transplantation.
