Corneal aberrations and total corneal refractive power after small incision lenticule extraction (SMILE): a 3-year follow-up study

Abstract Details

Purpose:

To evaluate changes in corneal aberrations and total corneal refractive power 3 years after small incision lenticule extraction (SMILE).

Setting:

Department of Ophthalmology, Aarhus University Hospital, Denmark.

Methods:

The study included 87 eyes from 87 patients treated for moderate to high myopia with SMILE (-6 to -10 diopters). Postoperative 3-month and 3-year examinations were made with Pentacam HR (Pentacam HR, Oculus, Wetzlar, Germany). Root-mean-square (RMS) of spherical aberration (Z,4,0), RMS of coma (Z,3,1 and Z,3,-1), and RMS of higher-order aberrations (HOA, from 3rd and up to 8rd order) were calculated in corneal front, corneal back, and for the combined corneal surfaces. Total corneal refractive power (TCRP) was evaluated in a 4-mm apex-centered zone. Changes from 3-month to 3-year examinations were calculated and analyzed with paired t-tests/Wilcoxon Signed-rank t-test.

Results:

RMS of spherical aberration decreased significantly from 3 months to 3 years in corneal front (-0.096 ± 0.01 μm), corneal back (-0.01 ± 0.01 μm) and the combined corneal surfaces (-0.02 ± 0.01 μm). No significant changes were seen in RMS of coma (p=0.11), but a decrease in RMS of HOA was significant in both corneal front and the combined corneal surfaces (-0.04 ± 0.01 μm, -0.03 ± 0.01 μm). TCRP increased significantly with 0.36 ± 2.9 D from 3 months to 3 years after surgery.

Conclusions:

The study showed slight decrease in corneal aberrations 3 years after SMILE, although no changes were seen in coma aberration. The increasing TCRP over time suggests a small myopic regression over the years after surgery.
