Comparison of the Pentacam AXL axial length and keratometry measurements with the IOLMaster 500 in intraocular lens power calculations for cataract surgery

Purpose:
To compare the accuracy of the Pentacam AXL equivalent axial length and keratometry measurements with the IOL Master 500.
Setting:
Department of Ophthalmology, Goethe-University, Frankfurt, Germany.
Methods:
153 phakic eyes scheduled for cataract surgery were enrolled in this retrospective study. The examined patients were 66 ± 12 years old, 46% right eyes and 54% left eyes. All eyes were first measured with the IOL Master 500 and then with the Pentacam AXL. Besides the axial length also the corneal radius, astigmatism and anterior chamber depth were measured with both devices in every patient. The obtained axial length measurements from each device were statistically compared by Bland-Altman plots and paired descriptive statistics.
Results:
Mean values in 153 phakic eyes with SNR > 3.5 were as follows: Mean difference in axial length measurements of Pentacam AXL and IOL Master 500 were - 0.001 ± 0.07 mm. In Bland-Altman plots the mean of axial length showed an upper limit of agreement of 0.14 mm and a lower limit of - 0.14 mm.
Analysis of the corneal radius, astigmatism and anterior chamber depth for all measurements showed: The mean difference was 0.02 ± 0.08 mm (Rflat); 0.03 ± 0.08 mm (Rsteep); 0.03 ± 0.07 mm (Rm), - 0.08 ± 0.37 mm (Astigm.) and 0.09 ± 0.17 mm (ACD).
Conclusions:
The Pentacam AXL device enables to perform accurate biometric measurements for axial length. For corneal radius, astigmatism und anterior chamber depth measurements the Pentacam AXL gave accurate results with only a small deviation from the IOL Master measurements.
