Retrospective study of Ahmed glaucoma valve implantation with tube insertion through the ciliary sulcus in uncontrolled glaucoma

Purpose:
To report the efficacy and safety of Ahmed Glaucoma Valve (AGV) insertion into the ciliary sulcus in pseudophakic patients and in combined surgery with cataract extraction.
Setting:
Assaf Harofeh Medical Center, Zerifin, Israel
Methods:
A Chart review was done. Main outcome measures were safety, IOP reduction and lowering of anti-glaucoma medication. Anterior segment imaging was used to document the position of the tube.
Results:
Nine consecutive eyes of nine patients were recruited with a mean follow up of one year.
Seven eyes were Pseudophakic, and 2 underwent combined cataract extraction with AGV implantation.
The mean age was 74 ± 8.1 years. Preoperative IOP was reduced from 25.5 ± 1.7 mm Hg to 10.7 ± 4.7 mm Hg. Antiglaucoma medications were reduced from 2.2 ± 0.9. to 0.13 ± 0.4 .One case of transient hypotony and a mild hyphema was documented, no other complications were seen . There were no cases of corneal decompensation or graft failure during follow up.
Conclusions:
Ciliary Sulcus placement of the tube of AGV seems a safe and effective alternative to the conventional technique with tube in the anterior chamber or the pars plana. Further comparative studies are required to demonstrate its superiority in terms of corneal protection.
