Bilateral inferior iris coloboma with glaucoma as a presentation of Axenfeld-Rieger syndrome

Abstract Details

Purpose:

To describe a clinical case of a patient with bilateral inferior iris coloboma, glaucoma and cataract that was diagnosed with Axenfeld Rieger Syndrome.

Setting:

Department of Ophthalmology, Hospital Garcia de Orta, E.P.E.

Methods:

Case report of a single patient.

Results:

A 35-year-old man presented to us with a history of progressive bilateral loss of vision. He has a longstanding history of intellectual disability and dental abnormalities. His visual acuity was 20/200 bilaterally and his intra-ocular pressure (IOP) was 25 mmHg in the right eye and 32 mmHg in the left eye. Anterior segment examination revealed bilateral inferior iris colobomas and cataracts. A presumptive diagnosis of Axenfeld Rieger Syndrome was made and he was treated with topical antiglaucomatous therapy that was unsuccessful in achieving IOP control. He was submitted to cataract and glaucoma surgery of the right eye with good control of IOP and increase in visual acuity to 20/100.

Conclusions:

We present a case of a patient that was diagnosed with Axenfeld Rieger Syndrome due to a decrease in vision caused by cataracts and glaucoma in his thirties. An early screening for these patients is important so that the ocular complications can be managed before they cause irreversible damage to visual acuity.
