Assessment of the characteristics of the filtration bleb measured by OCT and intraocular pressure changes after subconjunctival injection of 5- fluorouracil in trabeculectomy

Purpose:
To evaluate the efficacy and changes in the bleb after injection of 5-fluorouracil (5-FU) as a measure to modify the healing process and improve the outcome of glaucoma surgery based on trabeculectomy.
Setting:
Glaucoma Department, Inistituto universitario Barraquer, Universidad autónoma of Barcelona, Barcelona, Spain.
Methods:
19 eyes of 19 patients were included in this consecutive case series study. The patients underwent trabeculectomy augmented with mitomycin C.
The variables of the study were: the number of injections (NI), intraocular pressure (IOP), the height of the bleb (HB), the thinnest point of the conjunctive (HC) both measured with anterior segment OCT before and one week after then injection of 5-FU and complications in the process.
Results:
Mean NI was 1.3 +/- 0.6. Prior to the first injection IOP was 18.4 +/- 5 mm Hg and one week after the injection was 14.9 +/- 3; prior to the second injection IOP was 21.6 +/- 11 and one week after was.15.6 +/- 3 mmHg.
After the first injection of 5-FU the HB was 0.91 +/- 0.34 mm and improved to 1.06 +/-0.50 mm, and the HC was 0.31 +/- 0.2 mm and improved to 0.36 +/- 0.15 mm,
After the second injection the HB was 0.11 +/0.1 mm and changed to 0.13 +/- 0.36 mm, and the HC was 0.3 +/- 0.11 mm and changed to 0.4+/- 0.12 mm.
There were no complications related to the use of 5-FU.
Conclusions:
The use of 5-FU improved the structure and function of the filtering bleb, the correlation between the improvement of the structure characteristics and the IOP lowering was better after the first injection than the second injection.
