Relationship between visual field changes and optical coherence tomography measurements in advanced open-angle glaucoma

Abstract Details

Purpose:

To evaluate the relationship between functional changes in visual field and structural changes established by spectral-domain optical coherence tomography (SD-OCT) in advanced open-angle glaucoma (OAG).

Setting:

Department of Ophthalmology, Medical University Plovdiv, Bulgaria

Methods:

Thirty one eyes of 25 patients with OAG (mean age 63.1±10.9 years) are included in this study. Except routine ophthalmological exam the patients undergo standard automated perimetry – SAP (Humphrey Field Analyzer) and SD-OCT (RTV-ue – 100), done within 6 months. The global perimetric indices in the study group are as follows: Mean Deviation (MD) – 12.33±6.18dB and Pattern Standard Deviation (PSD) 9.17±3.41dB. The relationship between OCT measurements and the global indices MD and PSD is established by correlation (Pearson’ correlation coefficient) and regression (linear and nonlinear regression models) analyses.

Results:

The measurements of the lower halves of ganglion cell complex (GCC), retinal nerve fiber layer by scanning protocol ONH – RNFL 1 and retinal nerve fiber layer by scanning protocol 3.45mm – RNFL 2 (76.75±17.84 µm; 77.63±17.64 µm, 72.29±15.55 µm) present smaller thickness in comparison with those of the upper halves of GCC, RNFL 1 and RNFL 2 (80.20±21.95µm, 78.34±17.64µm, 73.87±14.60µm respectively), however without statistically significant difference. On the base of regressive analysis it is found that GCC correlates better with RNFL 1 (R² = 0.572, P<0,001) than with RNFL 2 (R² = 0.301, P=0,011). Correlation between global indices MD/PSD and most examined quantitative OCT measurements are moderate (with R between 0.3 and 0.6).

Conclusions:

The association between MD and GCC-Ave shows nonlinear cubic regression (R2 = 0.417, P=0.004). Good relationship is found between MD and GLV (R2 = 0.383; P=0.008). Linear regression (with P<0.05) is estimated only between MD and Cup Area (R2 = 0.175, P=0.024) and between MD and RNFL 2 (R2 = 0.131, P=0.045). Nonlinear regressive models appear to be more appropriate in evaluation of relationship between functional and structural changes in eyes with advanced glaucoma. The moderate correlation is established in this study.
