Effect of phacoemulsification on intraocular pressure in medically controlled open-angle glaucoma

Abstract Details

Purpose:

To evaluate the effect of phacoemulsification on intraocular pressure (IOP) in medically controlled eyes with open-angle glaucoma.

Setting:

Department of Ophthalmology, University Clinical Centre Maribor, Maribor, Slovenia

Methods:

This retrospective analysis included 124 consecutive eyes with medically controlled open-angle glaucoma in which phacoemulsification was done and the follow-up was 24 months in all eyes. The IOP was measured before and one week, 1, 3, 6, 12, 18, and 24 months after the phacoemulsification in all eyes. In all eyes at all follow-up visits the antiglaucoma medication remained unchanged. During the follow-up no eye showed any progression of visual field and optic disc damage requiring antiglaucoma surgery or laser surgery. 59 eyes had exfoliative glaucoma (XFG) and 65 eyes had primary open-angle glaucoma (POAG).

Results:

The mean IOP before surgery was 15.5 mmHg (SD 2.3) in the XFG group and 15.7 mmHg (SD 1.8) in the POAG group. The mean number of antiglaucoma medications was 1.64 (SD 1.2) in the XFG group and 1.89 (SD 0.9) in the POAG group. In the XFG group at all follow-up visits after the surgery, the mean IOP was significantly lower than the mean IOP before surgery. In the POAG group the mean IOP was significantly lower at 3 and 12 months and no significantly lower at all follow-up visits after the surgery. The difference in the mean IOP after the surgery between the XFG and POAG group was significant at all follow-up visits.

Conclusions:

Phacoemulsification in eyes with medically controlled open-angle glaucoma resulted in a reduction of IOP, which was greather in eyes with XFG than in eyes with POAG.
