The effect of cataract surgery on intraocular pressure and central corneal thickness in primary angle-closure glaucoma

Abstract Details

Purpose:

To investigate the effect of cataract surgery on intraocular pressure (IOP), central corneal thickness (CCT) and the number of anti-glaucoma medications in cases with primary angle-closure glaucoma (PACG).

Setting:

Twenty-one eyes of 21 cases with PACG were included to our prospective study which was conducted from January 2014 to October 2014 in Ankara Ulucanlar Eye Research Hospital

Methods:

The mean age of 14 female and 7 male cases was 67.5±5.02 (58-75 years). All of the cases had uncomplicated phacoemulsification and posterior chamber intraocular lens (PC IOL) implantation. In addition to complete ophthalmological examination including IOP by Goldmann applanation tonometer, CCT measurements were done by ultrasonic pachymeter before the surgery and at the first week, first month and third month postoperatively. Pre and postoperative values of IOP, CCT and the number of anti-glaucoma medications were compared statistically by t test, Wilcoxon and paired sample t tests.

Results:

The mean IOP was 20.8±2.9 mmHg and the mean CCT was 523.09±15.9 µm preoperatively. The IOP decreased to 15.8±1.4 mmHg at the first week (p=0.0001), 15.7±1.2 mmHg at the first month (p=0.0001) and 15.3±1.2 mmHg at the third month (p=0.0001). The mean number of medications was 2.4±0.7 preoperatively. It decreased to 0.3±0.5 at the first week and month (p=0.0001) and 0.4±0.5 at the third month (p=0.0001). The differences of IOP and the number of medications between postoperative first week, first month and third month weren’t significant. Also the differences between preoperative and postoperative values of CCT weren’t significant.

Conclusions:

Uncomplicated phacoemulsification and PC IOL implantation surgery can cause significant decreases in IOP and the number of anti-glaucoma medications in PACG cases. So cataract surgery alone can be a treatment option for PACG cases.
