New algorithm of high frequency femtosecond laser system (LDV Z8) for cataract surgery

Abstract Details

Purpose:

To report our clinical results using the new Z8 laser system for laser assisted cataract surgery (FLACS), inclusively clear corneal incisions regarding precision and safety using this new technology.

Setting:

Swiss Eye Research Foundation, Eye Clinic ORASIS, Reinach, Aargau, Switzerland

Methods:

Prospective case series. 248 patients were subjected to unilateral FLACS and 25 patients combined with clear corneal incision. Capsulotomy and lens fragmentation inclusively 25 cases clear corneal incision were performed with the aid of LDV Z8 femtosecond laser system. Ease of phacoemulsification (in 4-grade scale), completeness of capsulotomy, time for preparation of femtosecond laser (minutes), effective phacoemulsification time (seconds), quality of clear corneal incisions, total time of surgery (minutes) and safety of the procedure were evaluated.

Results:

Ease of fragmentation and completeness of capsulotomy was excellent (free float). There were no tissue bridges in the clear corneal incision from the first cut. The preparation time for the femtosecond was 3.6±0.7 min, effective phacoemulsification time was 1.8±2.4 sec for cataract grad 1-4 and 0.8±0.6 sec for cataract grad 1-3. Total time of femtosecond-assisted cataract surgery procedure was 18.8±5.0 min in the first session and from the 5th session 12.4±0.9. A steep learning curve was seen. No major complications were observed.

Conclusions:

FLACS with the LDV Z8 system was characterized by a complete capsulotomy, a highly effective and reproducible lens fragmentation and clear corneal incisions without tissue bridges. The safety level of the procedure was very good as it was perceived from the surgeon operating in this observational case series.
