Factors predicting corneal astigmatism reduction following femtosecond laser intrastromal astigmatic keratotomy

Abstract Details

Purpose:

To investigate predictors of corneal astigmatism reduction following femtosecond laser non-penetrating intrastromal astigmatic keratotomies (FSAK).

Setting:

Moorfields Eye Hospital, London United Kingdom

Methods:

323 eyes with corneal astigmatism underwent femtosecond laser-assisted cataract surgery with concurrent correction of astigmatism using paired symmetrical non-penetrating FSAK. FSAK were created following laser capsulotomy and lens conditioning following a nomogram (femtoemulsification.com). FSAK incisions were created at between 20% to 80% depth from the anterior corneal surface. All treatments were performed using an 8.0 mm optical zone with limbal centration. Follow up was performed at least one month postoperatively.

Results:

Univariable regression analyses showed anterior chamber depth, preoperative corneal cylinder, and FSAK arc length and depth to be associated with surgically induced refractive change (SIRC). Multiple variable analyses showed ORA CH and CRF, preoperative cylinder, astigmatism type (against/ with-the-rule/oblique), FSAK arc length and depth to be independent predictors of SIRC.

Conclusions:

This study suggests corneal biomechanical parameters and astigmatism type are independent predictors of the magnitude of astigmatism reduction using FSAK.
