Impact of lens densitometry values using Pentacam Scheimpflug system on ultrasound energy in femtosecond laser-assisted cataract surgery

Purpose:
In this study we aim to assess the correlation between lens densitometry measurements using Pentacam Scheimpflug Imaging System with equivalent average ultrasound power and effective phacoemulsification time (EPT) in femtosecond laser-assisted cataract surgery (FLACS).
Setting:
Department of Ophthalmology, Goethe-University, Frankfurt, Germany.
Methods:
This is a retrospective, non-randomized, comparative case series study.
124 eyes of 85 patients were included, where 107 underwent FLACS and 17 underwent refractive lens exchange (RLE).
Using the Pentacam Nucleus Staging (PNS) and lens densitometry program provided by the Pentacam Scheimpflug, preoperative mean lens density and cataract grading score were evaluated. Pupil diameter was above 4mm.
Surgical technique was standardized in all eyes and was performed by the same surgeon (T.K) using Alcon Centurion Vision System. The average ultrasound power and EPT were routinely recorded during surgery and correlated with the PNS grading and average lens density value.
Results:
Preoperative average lens density compared with EPT showed a weak yet significant positive linear correlation (P = 0.00). Overall, EPT increased as the lens density increased. This direct relationship was also noticed when EPT was compared with the PNS grading, demonstrating a significant moderate correlation (P = 0.00). With increasing average lens density, the equivalent average ultrasound power showed a positive correlation and was also found to increase in value (P = 0.00). A total of 39 cases, including 37 with a PNS grade between 0 to 1 (scale range from 0 to 5), did not require ultrasound energy during phacoemulsification.
Conclusions:
FLACS has been shown to correlate positively with the Pentacam Scheimpflug System that offers an objective and repeatable measure of the lens density and cataract grading. By quantifying preoperative lens density values and cataract nuclear staging, intraoperative phacodynamics can be predicted and altered to reduce phacoemulsification complications including corneal endothelial injury.
