Early clinical effects on moderate and high myopic astigmatism following SMILE

Purpose:
To evaluate the therapeutic effect of moderate and high myopia with astigmatism after small incision lenticule extraction (SMILE).
Setting:
Department of Ophthalmology, Institute of Field Surgery, Daping Hospital, Third Military Medical University, Chongqing 400042, China
Methods:
47 patients (92 eyes) high myopic astigmatism were treated with SMILE, there were 54 eyes with moderate astigmatism (-1.00D~-3.00D) in group A, and 38 eyes with high astigmatism (-3.00D~-4.50D ) in group B. The operation were performed with the VisuMax femtosecond laser, intended cap thickness was set 120-130μm respectively with 2mm of side cut. Refractive outcomes were evaluated through uncorrected visual acuity (UCVA), best corrected visual acuity (BCVA), refractive powder, residual powder in astigmatism and visual contrast sensitivity at 1day、1week、1monts and 6 months after surgery.
Results:
There were 92 eyes completed according to the design procedure, including 3 eyes with obvious opaque bubble lays, 1 eyes with black spot. Postoperative UCVA is equal or more than preoperative BCVA: 89, 90, 92,79 (only 79 eyes were received the examination at 6mo)eyes at 1d, 1wk, 1mo and 6 mo after operation respectively, furthermore postoperative UCVA is gained two lines comparing with preoperative BCVA: 14, 18, 22, 16 eyes at 1day, 1week, 1month and 6 months after the operation respectively. There was no significant difference in UCVA and contrast sensitivity after surgery between moderate and high astigmatism group.
Conclusions:
SMILE is the safe, effective, and predictable surgical procedures to treat moderate and high myopic astigmatism. Centration of optic zone and water infiltration separation technology could improve the accuracy of refractive outcomes.
