Pneumococcal keratitis treated with ultraviolet A/riboflavin corneal cross-linking: a case report

Abstract Details

Purpose:

We described herein a case of keratitis caused by the Streptococcus pneumoniae treated with UVA-riboflavin crosslinking.

Setting:

Selcuk University, Faculty of Medicine, Department of Ophtalmology

Methods:

A 28-year-old man who had a penetrating keratoplasty surgery 6 months before presented with a 1-week history of pain, decreased vision, photophobia, foreign body sensation, and lacrimation in the right eye. On biomicroscopic examination, he had intense conjunctival injection, a large central corneal infiltrate and peripheral vascularization. The corneal scrape was performed and the specimen was sent for gram stain and culture. In vivo confocal microscopy showed round inflammatory cells in the epithelium and stroma. He was started on hourly topical ceftazidime and vancomycin and systemic moxifloxacin, but the keratitis did not improve. The culture was positive for Streptococcus pneumoniae.

Results:

UVA-riboflavin corneal crosslinking was performed and one day after the procedure, the patient reported a significant improvement in symptoms. One month after the treatment, keratitis almost completely regressed and corneal ulceration was healed.

Conclusions:

UVA-riboflavin crosslinking can be useful for the treatment of keratitis unresponsive to medical treatment.
