Current protocols of corneal collagen cross-linking: visual, refractive and topographic outcomes

Abstract Details
Purpose:
To study the effect of different protocols of collagen crosslinking on visual,refractive and topographic parameters in patients with progressive keratoconus.
Setting:
Narayana Nethralaya Superspeciality Eye Hospital and Postgraduate institute of Opthalmology,Bengalore,India
Methods:
In this prospective, interventional study, 172 eyes of 172 patients with progressive keratoconus underwent corneal collagen crosslinking (CXL). Following detailed preoperative examination, Group I underwent conventional crosslinking (36 patients, 3mW/cm2) while groups II (36 patients, 9mW/cm2), group III (33 patients, 18mW /cm2), Group IV (33 patients, 30mW/cm2) and Group V (34 patients, 45mW/cm2)
underwent accelerated crosslinking. Epithelium was debrided in groups I - IV while patients in group V underwent a transepithelial CXL. Changes in CDVA, SE, flat K, steep K, thinnest pachymetry, specular microscopy and demarcation line were studied.
Results:
Improvement in mean CDVA and SE were statistically significant in all groups except group IV (p=0.15, p=0.17) with group III (p=0.02) showing the best results. Flattening of steep and flat keratometry was significant in group I (p<0.001) and II (p=0.002) as compared to the other groups. There was no significant difference in the pachymetry or specular microscopy change in any of the groups. Group I and II demonstrated a good demarcation line when compared to other groups.
Conclusions:
Conventional CXL (group I) and accelerated CXL with irradiations of 9mW/cm2 (group II) and 18mW/cm2 (group III) showed better visual, refractive and topographic improvements at the end of 12 months.
