Efficacy of trans-epithelial corneal collagen cross-linking for keratoconus

Abstract Details

Purpose:

Purpose was to assess the efficacy of transepithelial (TE) corneal collagen crosslinking for the treatment of keratoconus.

Setting:

Ebsar eye center, Benha, Egypt

Methods:

The study included 30 eyes from 18 patients treated by transepithelial CXL. The mean age was 25.2 years. Preoperative and postoperative examinations included corrected (CDVA) distance visual acuities, keratometry (K), and corneal indicies were evaluated. The solution for transepithelial CXL (Avedro) comprised of ParaCelTM ( riboflavin 0.25%, HPMC, NaCl, EDTA, Tris, and benzalkonium chloride) , and vibeX-Xtra (riboflavin 0.22%, phosphate buffered saline solusion) . Ultraviolet-A treatment was performed with the Xlink system (OptoXLink, Opto Global Pty Ltd, Australia ) at 3 mW/cm2 for 30 min. Postoperative follow-up was 1 day,1, 3, and 6 months.

Results:

This prospective study evaluated 30 eyes of 18 patients with a mean age of 25.2 ± 6.19 years, ranged (17 – 38) years. The mean logMAR of CDVA (0.54 ± 0.21 preoperatively versus 0.61 ± 0.19 at 6 months postoperatively) (P<.001). The mean spherical equivalent refractive error was -6.61 ± 3.90 diopters (D) preoperatively and -5.91 ± 3.72 D at 6 months postoperatively (P<.001). The mean preoperative corneal astigmatism was, -3.01± 2.64 D and at 6 months postoperatively -2.46 ± 2.60 D (P<.001). The mean maximum K value flattened by 1.0 ± 0.87 diopter.

Conclusions:

Transepithelial CXL resulted in a statistically significant improvement in maximum K values and CDVA at the 6-month follow-up. Further follow-up is necessary to ascertain the ability of transepithelial CXL to achieve long-term stabilization of the cornea in eyes with keratoconus.
