Pentacam topography indices after cross-linking for different stages of primary keratoconus

Abstract Details

Purpose:

To evaluate the results of corneal collagen cross-linking (CXL) for different stages of primary keratoconus using Pentacam topography indices.

Setting:

S. Fyodorov IR&TC “Eye Microsurgery”, St.-Petersburg, Russian Federation

Methods:

The study comprised 32 patients (61 eyes). Treatment group consisted of two subgroups depending on the stage of disease: from mild to moderate keratoconus in the first subgroup and advanced keratoconus in the second subgroup. The control group consisted of patients with fruste-form keratoconus without operations. Average follow-up was 1.5 years (18.5 ± 5.5 months). Corneal collagen cross-linking was performed by standard method, using Russian UV-device and solution of riboflavin. Corneal thinnest point, back elevation, astigmatism, topography indices ISV, IVA, KI, CKI, IHA, IHD and Rmin were measured by Pentacam. For statistical analyze paired Wilcoxon test was used.

Results:

In both treatment subgroups a significant decrease in ISV, IVA, KI, CKI, IHD was found. ISV decreased more than others. IHA was not changed and Rmin was increased (p<0.05). Severity of keratoconus and uncorrected distance visual acuity were decreased (p<0.05). Corrected distance visual acuity was not changed, but tended to increase. Spherical and cylindrical refractive components of refraction were decreased in patients of the first subgroup and were not changed in patients of the second subgroup (p<0.05). In the control group there were no significant changes in the studied parameters except spherical component of refraction.

Conclusions:

Five of seven Pentacam topography indices are significantly decreased after CXL regardless of keratoconus stage. It leads to improvement of refractive parameters. ISV is the most informative index for evaluation of operating result.
