Phototherapeutic keratectomy versus manual epithelial debridement for corneal collagen cross-linking: 1 year outcomes of 281 eyes with progressive corneal ectasia

Purpose:
To evaluate and compare the long-term effectiveness of mechanical epithelial debridement using a manual brush versus transepithelial phototherapeutic keratectomy (PTK) combined with corneal collagen cross-linking (CXL) in eyes with progressive corneal ectasia
Setting:
Ottawa Eye Institute,The Ottawa Hospital, Ottawa, Ontario.
Methods:
Retrospective review of 330 eyes with progressive corneal ectasia undergoing CXL combined with either mechanical epithelial debridement (“brush”) (n = 258) or PTK (n = 72 eyes).
Results:
There was a significant improvement in all refractive and topographic outcomes (i.e. UDVA, BCVA, sphere, cylinder, K mean, K flat, K steep, K max and pachymetry values) in all CXL eyes at 12 months (p<0.005). There was a significant improvement in UDVA with eyes treated with PTK-CXL versus brush at 12 months. There was no difference in other refractive (CDVA, sphere, cylinder) or topographic outcomes (i.e. K mean, K flat, K steep, K max) between the two groups.
Conclusions:
Corneal collagen cross-linking significantly improves all refractive and topographic data with either PTK or manual epithelial debridement at 12 months post-operatively. Both techniques are comparable in refractive and topographic outcomes, except for UDVA, which was better in the PTK group compared to the brush group.
