Intrastromal corneal ring segment implantation for mild to moderate keratoconus, refractive and topographic outcomes

Abstract Details

Purpose:

To assess the refractive and topographic outcomes of intra stromal corneal ring implantation (Ferrara®) in patients with mild to moderate keratoconus, as well as accuracy of the laser tunnel using optical coherence tomography (OCT SD) and fitting contact lens to achieve best corrected visual acuity postoperatively.

Setting:

Although rigid gas permeable contact lenses correction is satisfactory for most patients, management of this irregular astigmatism remains a challenge when contact lenses are not well tolerated. For such cases, intrastromal corneal rings implantation was proposed for improving uncorrected and corrected distance visual acuity. Retrospective Study, Eye Center in Paris.

Methods:

Patients with mild to moderate keratoconus (Km ≤ 58 diopters),and Rigid contact lens intolerance were followed at 1 month, 3 months, 6 months and 1 year after implantation of Ferrara intrastromal corneal ring segment. The preoperative and postoperative data were collected: Uncorrected Distance Visual Acuity (UDVA), Corrected Distance Visual Acuity (CDVA), Spherical Equivalent (SE), corneal topography and depth of corneal tunnel using OCT SD. Intracorneal rings implantation was performed using the Intralase femtosecond laser.

Results:

27 patients were included. Mean age at surgery was 28.2 ± 10.3 years. CDVA converted to LogMAR improved from 1 ± 0.3 preoperatively to 0.25 ± 0.2 after surgery (p=0.002). The mean preoperative and postoperative spherical equivalents were -7.2 D ± 5.4 and -6.1 D ± 1.8 respectively (NS). The mean keratometric cylinder decreased from 4.3 D ± 3.9 to 2.5 D ± 2.1 (p=0.004). The OCT scans showed accurate architecture and depth of intra stromal tunnel.

Conclusions:

Intrastromal corneal ring implantation appears to be an effective technique for reducing astigmatism. OCT SD showed accurate measurement of tunnel depth. Contact lens fitting can improve CDVA.
