Refractive outcome after corneal graft with same size punch and trephine sets for treatment of keratoconus

Abstract Details

Purpose:

To evaluate refractive outcome of penetrating keratoplasty (PKP) and deep anterior lamellar keratoplasty (DALK) for keratoconus with same size punch and trephine sets.

Setting:

Eye research center, Feiz hospital, Isfahan University of Medical Sciences, Isfahan, Iran

Methods:

In this study, 35 patients underwent corneal transplantation by PKP and 35 patients by DALK. We used Katena Barron vacuum punch/trephine sets. Diameter of punch and trephine were chosen the same in all patients (7.75 or 8.00 mm according to surgeon preference). Standard PKP performed with 8 separate and 1 running 10-0 nylon stitches. Baring of Descemet’s membrane in DALK group carried out with big bubble technique. Patients evaluated 3 month after removal of all sutures regarding their vision and refractive error.

Results:

Median corrected acuity in both PKP and DALK groups were 0.5 decimal notation, we did not find significant difference between two groups. 17.1 % of PKP group and 25.7 % of DLK group had corrected acuity equal to or better than 0.8. 77.1 % in PKP group and 68.6 % in DALK group had acuity between 0.8 and 0.3. The difference between two groups was not statistically significant (P=0.83). Mean spherical equivalent refractive error in PKP and DALK groups was -3.86 +/- 3.72 and -2.78 +/- 4.19 diopters respectively. The difference was not statistically significant between two groups.

Conclusions:

Both PKP and DALK performed with same-size punch/trephine sets are acceptably effective methods for treatment of advanced forms of keratoconus. We were not able to disclose any difference regarding visual and refractive parameters between two groups.
