Long-term success with PTK followed by toric contact lenses in a case with gelatinous droplet dystrophy

Abstract Details

Purpose:

To report and discuss the results of PTK and toric contact lens fit in a young man with gelatinous droplet dystrophy(GDD).

Setting:

Department of Ophthalmology, Dokuz Eylul University, Izmir, Turkey.

Methods:

A 21-year-old man had a penetrating keratoplasty in April 2006 for GDD in the right eye and was referred to us for recurrence on graft. Visual acuity was counting fingers OD and 0.8 OS. Slit-lamp exam revealed GDD recurrence over the nasal half of the graft OD and a central lump of GDD OS. Intraocular pressures and fundus exam were within normal limits. PTK was performed OD after obtaining informed consent in September 2008.

Results:

Visual acuity improved to 0.6 with -3.0-4.0x10 OD and remained constant for 2 years. The patient noted a decrease in vision in both eyes in 2010. Visual acuity was 0.1 OD and 0.3 OS. Slit-lamp exam revealed a second recurrence OD and an increase in the area of GDD OS. Bilateral PTK was performed this time in June 2010. Visual acuity improved to 0.6 OD (-2.0-4.5x15)and 0.7 (-1.5-5.00x130)OS. Two more lines were gained with glasses over contacts and patient agreed to wear contacts and glasses. No complications were noted for 5 years of follow-up.

Conclusions:
