Incision modification for Descemet's stripping automated endothelial keratoplasty in phakic patients

Abstract Details

Purpose:

To report the results of a modified Descemet Stripping Automated Endothelial Keratoplasty (DSAEK) in phakic eyes with endothelial failure.

Setting:

Institution Villa Igea, Forli, or at a public hospital,Ospedale M.Bufalini, Cesena, Italy

Methods:

The charts of all patients with a clear crystalline lens and endothelial failure that underwent modified DSAEK at our institution between 2005 and 2014 were reviewed. The incision sites were moved from the standard 9 and 3 o’clock position in order to avoid the pass of intraocular instruments across the exposed crystalline lens. Postoperative cataract formation in these patients was compared to a cohort that included all the patients with a clear crystalline lens and endothelial failure that underwent conventional DSAEK at our institution during the study period. significant)

Results:

49 eyes had DSAEK using modified wound position (1), 35 eyes underwent DSAEK with conventional incision sites(2). Mean age was 35.7 years in the (1) and 44.8 ±16.1 in the (2). Follow-up time was 13.8±9.6 months for the (1) and 25.9±11.3 months for the (2). Cataract in 5 eyes (10.2%) of the (1), in 11 eyes (31.4%) of (2). An traumatic cataract developed within 4 months from surgery in 2 eyes (4.1%) of the (1) and in 7 eyes (20%) of (2). cataract Senile or steroid induced cataract was diagnosed in 3 eyes (6.1%) of (1) and in 4 eyes (11.4%) of (2). Traumatic cataract diagnosed in higher number of eyes of (2) in comparison with (1).

Conclusions:

The occurrence of traumatic cataract following phakic DSAEK may be reduced significantly by a basic modification in the location of the incision sites. Although also the total rate of cataract formation was higher in the control group, this could be the result of the older age and longer follow-up time of the patients in that group.
