Late term corneal and scleral dellen development and its treatment with autologous serum after pterygium surgery with conjunctival autograft

Abstract Details

Purpose:

We aimed to report a case with late term corneal and scleral dellen development after pterygium excision and conjunctival autograft implantation without mitomycin C, which was resistant to artificial tear and gel therapy and improved after autologous serum therapy.

Setting:

Kırsehir Ahi Evran University Faculty of Medicine, Training and Research Hospital, Ophthalmology Department, KIRSEHIR-TURKEY.

Methods:

A 52-year-old male patient without any systemic or ocular disorder was applied pterygium excision and conjunctival autograft implantation without mitomycin C under local anesthesia for nasal primary pterygium. He also had no history of eye surgery.

Results:

The graft was necrotic and liquefied on postoperative 11th day despite topical steroid, antibiotic, and artificial tear. There were corneal and scleral dellen. Topical steroid application was stopped and the frequency of artificial tear was increased (24x1). Corneal and scleral dellen did not regress on postoperative 15th day despite addition of lubricant gels/ointment. Autologous serum drops at a concentration of 30% was added to the existing treatment. Corneal dellen was completely healed and scleral thickening was observed on postoperative 22nd day.

Conclusions:

Corneal and scleral dellen may be encountered at late term, albeit rare, after pterygium excision and conjunctival autograft implantation without mitomycin C. A marked improvement can be achieved in a short period with autologous serum drops added to the regimen of cases resistant to topical lubricant drops and lubricant gels/ointment, as in the present case.
