First results of femto laser-assisted penetrating and deep anterior lamellar keratoplasty

Abstract Details

Purpose:

To estimate effective parameters of femto laser energy for penetrated and lamellar cornea cuts in patients with different cornea pathology and to assess postop results of penetrated (PK) and deep anterior lamellar keratoplasty (DALK).

Setting:

Multidisciplinary Science and Technology Complex Eye Microsurgery named after Academician S. N. Fyodorov, Federal State Institution, Novosibirsk Branch, Russia

Methods:

Penetrated and lamellar cornea cuts in patients and donor cornea were performed by using FEMTEC 520F (Technolas Perfect Vision). The study included 12 patients (12 eyes) with keratoconus, granular corneal dystrophy type I, superficial central leucoma, post-traumatic scar, endothelial dystrophy of cornea. The penetrated and lamellar keratoplasty laser programs were used for cutting of recipient and donor corneas. Diameter of recipient bed was less 200 micron, than the donor one. Different bed energy, bed spacing were applied for different type of cornea pathology. Post-op control included optical coherent tomography (OCT) of transplanted cornea, corneal topography, visual acuity.

Results:

The quality of penetrating and lamellar cuts with bed energy 900-1300 nJ and bed spacing 6,0-6,3 micron was good in all recipient and cold storage donor corneas. Cornea disks after laser cutting were easy removed without knight or scissor. The border and lamellar surface of laser cutting cornea were absolutely round and very smooth. Intraoperative adaptation of donor and recipient cornea was good in all cases that provided non-complicated surgery (penetrated and deep anterior lamellar keratoplasty). Near post-op OCT showed good approximation of transplanted and recipient cornea sutured with 10/0 nylon. The transplanted corneas were clean, transparent in all eyes. Corneal topography showed less induced astigmatism in patients with DALK. No difference in visual acuity between PK and DALK.

Conclusions:

Both the femto laser assisted PK and DALK techniques were effective in restoring a clear cornea with a good recovery and visual functions 6-9 months after surgery.
