Intrastromal corneal ring segments for keratoconus treatment: first experience using INTRASEG® ICRs

Abstract Details
Purpose:
To evaluate the outcomes, efficacy and safety of intrastromal corneal ring segment (ICRS) implantation for keratoconus treatment and describe our experience in a case series.
Setting:
Surgeries were performed at Instituto Zaldivar de Mendoza, Argentina.
Methods:
108 eyes of 78 patients with keratoconus were enrolled, all of them where operated with implantation of ICRS - Intraseg® (Gamma Vision, Argentina), using different range of segments height, in a 25 microns step from 100 microns up to 300 microns, tunnel was created using femtosecond laser technique (IntraLase®, Irvine California, USA) and were followed-up during one year. A complete ophthalmic examination was performed, including UDVA, CDVA, corneal keratometry and corneal thickness (Visante®, St. Paul, Minnesota, USA) the measurements were made preoperatively and at 1, 3, 6 and 12 months postoperatively, statistical analysis was performed.
Results:
The mean age at the time of ICRs intervention was 30.64 ranges 17 – 63 (SD 9.42). Mean UCVA (logMAR) changed from 1.11 ± 0.61 (SD) preoperatively to 0.54 ± 0.58 (SD) postoperatively (P<0.001), the mean BCVA was 0.52 ± 0.18 (SD) preoperatively and postoperatively 0.14 ± 0.17 (P<. 0001). The maximum K value decreased from 48.75D preoperatively range 41 – 62.25 to 46.18D (P<.0006) postoperatively, and the minimum K change from 44.69D to 43.00D, the Topcon cylinder value decreased from mean -5.19 (range -0.80 to -8.80) to -2.6 (range -1.00 to -7.25), No intraoperative or post operative complications occurred. All the results were statistically significant (SPSS for Windows).
Conclusions:
This study shows that Intraseg ICRs implantation is a safe and efficient procedure for the treatment of keratoconus improving visual acuity, reducing the astigmatism and spherical correction, improving topographical regularity due to a wide variety of choices using different range of segments height.
