Abstract Details
Purpose:
To evaluate the efficacy of femtosecond-assisted Intraseg intrastromal ring segment (ICRs) implantation combined with cross linkling in reducing high astigmatism in patients who have previously undergone deep anterior lamellar keratoplasty (DALK).
Setting:
Surgeries were performed at Instituto Zaldivar de Mendoza, Argentina.
Methods:
This retrospective, observational study included patients who had previously undergone DALK and after several controls of the surgery presented high residual astigmatism. Patients were implanted with Intraseg® ICRs (Gamma Vision, Argentina) using a femtosecond laser (IntraLase®) combined in the same procedure with cross linking. UDVA, CDVA, keratometrics, corneal thickness was measured preoperatively at 1, 3, 6, and 12 months postoperatively.
Results:
35 eyes of 28 patients (aged 42.24 ± 13.9 years) were analyzed. The mean time between corneal graft and ICRs surgery was 34 months. Mean UCVA (logMAR) changed from 1.22 ± 0.48 preoperatively to 0.19 ±0.14 postoperatively (p<0.001), and BCVA from 0.36 ± 0.19 to 0.11 ± 0.38 (p<.0001). Kmax decreased from 50.98 D preoperatively to 48.52 D (p<.0003) postoperatively, while Kmin changed from 42.91 D to 44.00 D. Mean cylinder decreased from -7.10 (range -3.50 to -10) to -4.2 (range -1 to -7). Mean pachymetry was 581± 51.6 (SD) preoperatively vs 592 ±45 (SD) postoperatively.
Conclusions:
Intraseg ICRs implantation combined with cross linking an efficient and safe procedure for the treatment of high astigmatism after corneal graft, producing topographical changes, regularizing astigmatism, with a better spherical equivalent improving the CDVA.
