Efficacy and safety of corneal collagen cross-linking at 5 years follow-up in patients with keratoconus

Abstract Details

Purpose:

To determine the efficacy and safety of corneal collagen crosslinking in eyes with keratoconus, with a follow-up of at least 5 years.

Setting:

Centro Oftalmologico Virgilio Galvis

Methods:

Medical records of 59 eyes of 34 patients (20 men, 14 women) with keratoconus that underwent crosslinking with a minimum follow-up of 5 years were analyzed. Parametric hypothesis tests and non-parametric bivariate analyzes were made.

Results:

Spherical equivalent showed no statistically significant change before and 5 years after surgery: -4.51 ± 3.24 diopters (D) and -4.5 ± 3.22 D respectively (p = 0.251). Mean keratometry showed a nonsignificant decrease: 48.76 ± 5.18 D to 48.36 ± 5.34 D (p = 0.071). The corrected visual acuity (AVCC) improved from 20/40 to 20/36 (p = 0.038). 13 eyes (20.7%) lost 1 or 2 lines of vision. Endothelial cell count 5 dropped by 3.9% (p = 0.0172), but considering the physiological annual loss of 0.6%, an additional 5 years endothelial loss of only 0.9% was estimated.

Conclusions:

The findings of this study suggest that crosslinking is a safe and effective alternative in halting the progression of keratoconus at least 5 years of follow-up. However a percentage of eyes may show loss of lines of corrected vision.
