How successful is the surgical treatment of keratoconus using ICR vs the conservative treatment of keratoconus using RGP

Purpose:
To evaluate the outcomes of intrastromal corneal ring segment (ICRS) Implantation for the treatment of keratoconus versus the use of rigid gas permeable contact lens (RGP) based on the preoperative visual impairment of the patient and grading of keratoconus.
Setting:
This non randomized retrospective study comprised 25 patients with bilateral progressive keratoconus treated with the ICR and 25 patients with bilateral progressive keratoconus treated with the RGP. The 50 eyes were subjected to cross linkage performed by applying an enhanced riboflavin solution 0.1%, dextrane.

Methods:
Uncorrected and best corrected visual acuity (UCVA, BCVA), refraction, corneal topography parameters were evaluated at baseline and at 3, 6 ,12 months and 24 months. A two-year follow-up analysis of patients with stable and progressive keratoconus is performed in order to assess the long term stability of the surgical procedure
Results:
Corrected distance visual acuity decreased statistically significantly in patients with mild keratoconus (P <0.01) but statistically significantly increased in all other grades (P <0.05 in patients performing ICR procedure, however Corrected distance visual acuity statistically significantly increased in all other grades (P <; 0.05)in patients using RGP contact lens. The improvement in visual acuity and the decrease of keratometric and aberrometric values were stable throughout a long period of time in patients with stable keratoconus.
Conclusions:
In patients with progressive form keratoconus, a significant improvement was found immediately after the ICR and RGP, however clinically relevant regression greater than 3 D was observed at the end of the follow up period in patients performing ICR.
