Prevalence of guttae in donor tissue after transplantation

Abstract Details
Purpose:
To evaluate the prevalence of guttae in donor tissue following penetrating keratoplasty (PK) or Descemet stripping automated endothelial keratoplasty (DSAEK) and to examine their possible effect on postoperative results.
Setting:
Retrospective cohort study of eyes operated at a hospital by a single surgeon (MB).

Methods:
All recorded eyes undergone PK and DSAEK at Villa Igea Hospital from January 2005 till July 2014 were reviewed.We measure the postoperative presence of central guttae with specular microscopy.Indications for surgery, surgery type, postoperative BSCVA,and specular microscopy results were noted. Preoperative donor age and graft endothelial cell counts were recorded.Patient’s age,as well as postoperative BSCVA and ECD obtained at least 6 months postoperatively were compared between eyes with postoperative guttae and a cohort of eyes which didn’t demonstrate guttae.A two-tailed Student’s t-test was used for quantitative analysis and Fishers’ exact test for the analysis of categorical variables.
Results:
A total of 11 , 068 postoperative specular microscopy photographs were available for 1116 of 2332 (47.9%) eyes that underwent keratoplasty during the study period . Guttae were identified in 45 /1116 (4%) of these eyes. Guttae were noted in 42 of 946 (4.44%) of eyes following endothelial replacing procedures 27 photos demonstrated a few isolated scattered guttata, 9 showed widespread guttata with small patches and 9 demonstrated large patches of guttata . Prevalence of postoperative guttae was higher after PK than after DSAEK (p = 0.0001). After DSAEK, prevalence of guttae didn’t differ between Fuchs’ eyes (12/347 = 3.46%) and non-Fuchs’ eyes (p = 0.39) . BSCVA didn’t differ between patients with or without guttata ( p = 0.25).
Conclusions:
Postoperative guttata are could be found in 4% of post-keratoplasty grafts. The presence of guttae in the graft does not affect visual results or graft survival in the early till medium-term postoperative course.
