Factors associated with reduction in total and higher order aberrations in keratoconic eyes fitted with rose-K lenses

Abstract Details

Purpose:

Rose K lenses correct the irregular astigmatism of the anterior corneal surface in keratoconic eyes. The present study was designed to study the changes in total and higher order aberrations (HOA) and factors associated with these changes, in keratoconus patients fitted with Rose-K contact lenses.

Setting:

This study was conducted in Laxmi Eye Institute, Uran Road, Panvel, India

Methods:

We recorded total and higher order aberrations (spherical aberration, coma, trefoil, secondary astigmatism) before and after fitting of Rose-K contact lenses in 21 eyes of 17 patients diagnosed with keratoconus. The medians (and range) were compared using the Wilcoxon Sign Rank test. We used random effects linear regression to evaluate the factors associated with change in aberrations

Results:

There was significant reduction (median [range]) in total and HOA post Rose-K (Total: 2.957 [0.560, 21.581] vs 0.244 [0.171, 2.846]; p=0.0001; HOA: 1.243 [0.335, 19.530] vs 0.158 [0.040, 2.253], p=0.0001). Patients with SE < -5 had significant reduction in total (-6.590, 95% CI: -10.753, -2.428) and HOA (-4.841, 95% CI: -8.825, -0.856) compared with those whose SE was between 0 and -2. Patients with spherical errors < -5D were significantly more likely to have reduction in trefoil (-2.061, 95% CI: -2.670, -1.451) and secondary astigmatism (-1.374, 95% CI: -2.311, -0.436) compared with those whose errors were between 0 and -2D

Conclusions:

Rose K lens was useful in significant reduction of total and all the HOAs. The reduction in aberrations was more pronounced in people with higher spherical errors and those with high SE values
