Microbial keratitis in a referral hospital in Taiwan

Purpose:
To analyze predisposing factors, microbiological characteristics, and treatment of microbial keratitis in Taiwan.
Setting:
Chang Gung Memorial Hospital, a referral center in Taiwan.
Methods:
We retrospectively reviewed the medical charts of 558 patients who were diagnosed with microbial keratitis and admitted at our hospital from January 2003 to December 2012. Demographics, predisposing factors, isolated organisms, treatment and hospital bed days were recorded. Trends from year to year were tested by a linear-by-linear association.
Results:
In the 10-year study period, Pseudomonas aeruginosa was the most isolated organisms (28%), followed by fungi (17.6%) and coagulase-negative Staphylococcus (5.4%). The trend of Serratia marcescens was significantly decrease (p= 0.033). Contact-lens wear was the most common predisposing factor (31.4%), followed by ocular and systemic disease (26.3%) and trauma (23.5 %). Contact lens related infectious keratitis increased year by year (p=0.011). Most bacterial infections were controlled by medical treatment, but more than half of the patients with fungal keratitis needed surgical interventions. The average stay in hospital was 13.7 ± 11.5 days. Ocular surgery, large ulcer size, nontuberculous Mycobacterium infection and surgery during admission were related to prolonged hospital bed days.
Conclusions:
Contact lens related Pseudomonal keratitis was the most common cause of hospitalized microbial keratitis in Taiwan from 2003 to 2012.
