Exposure keratopathy in critically ill patients admitted in medical intensive care unit (MICU)

Abstract Details

Purpose:

To determine the prevalence of exposure keratopathy in critically ill patients hospitalised in the intensive care unit and to identify factors that predispose to its development.To promote Awareness and emphasis on eye care in intensive care unit (ICU) patients has translated to eye assessment being part of routine care

Setting:

Tertiary hospital in rural part of South India, Narayana medical college and hospital, medical intensive care unit, department of Ophthalmology

Methods:

Prospective ,observational study,of 298 patients admitted in the medical intensive care unit for more than 24 hours between Feb-Dec 2014. The eyelid position,presence of conjunctival oedema, degree of keratopathy and sedation score were documented at every assessment.

Results:

Exposure keratopathy was present in 49(27.24%) patient of these 35 patients has bilateral and 14 had unilateral. Invasive ventilation was found to be highly associated with development of keratopathy 15.467 (p < 0.001).Use of sedatives 16.426 (p< 0.001) and muscle relaxants 19.646(p< 0.001) highly associated with its development.

Conclusions:

Critically ill patients often develop ocular surface disorders, especially when sedated and immobilized. ophthalmologic attention must be given to patients in intensive care units to prevent the development of keratopathy,Eyelid position and duration of ventilation are associated with exposure keratopathy.A need for increased awereness and training among paramedical and medical staff of exposure keratopathy,and be a part of there routine examination to prevent it.
