Tear meniscus measurement by spectral optical coherence tomography in rheumatoid arthritis patients

Abstract Details

Purpose:

To evaluate the use of lower tear meniscus measurements obtained with anterior segment Spectral Optical Coherence Tomography (OCT) in quantitative tear evaluation rheumatoid arthritis patients.

Setting:

Cornea and Refractive Surgery Department

Methods:

A total of 90 eyes of 45 rheumatoid arthritis patients, mean age 50.38 ±13.21 years, were enrolled. Each patient completed general ophthalmic examination. Lower tear meniscus was evaluated using Spectral OCT (RTVue, Optovue) with cornea-anterior segment lens short. Three parameters were measured: tear meniscus cross-section area (TMA, mm2), tear meniscus height (TMH, mm), and tear meniscus depth (TMD, mm). Break-up time and Schirmer tests after instillation of topical anesthetic drops were also evaluated.

Results:

The correlation with Schirmer test results was found with TMA and TMD measures. Spearman correlation coefficient values were 0.21 and 0.26 respectively. Correlation was significant at the 0.05 levels.

Conclusions:

Lower tear meniscus parameters measured with Spectral OCT correlated with the Schirmer test and subjective symptoms in rheumatoid arthritis patients. Spectral Optical Coherence Tomography may be an alternative confirmative method for visualizing the ocular dry eye diseases.
