Combined trabeculectomy and phacoemulsification surgery in patients with advanced glaucoma

Abstract Details

Purpose:

To evaluate the results of combined phacoemulsification and trabeculectomy surgery in patients with cataract and advanced glaucoma.

Setting:

Ankara Atatürk Training and Research Hospital, Turkey

Methods:

Thirty-two eyes of 32 patients with cataract and advanced glaucoma were included in this study. All patients had separate-site combined phacoemulsification and trabeculectomy under local anaesthesia. Patients were followed up at least 1 year. Preoperative and postoperative visual acuity (VA), intraocular pressure (IOP) and number of antiglaucoma medication were investigated. During surgery and at follow-up visits, complications and other treatment approaches (like needling or new surgery) were evaluated. Complete and qualified success rates (intraocular pressure less than or equal to 18 mmHg with or without medication) were analyzed.

Results:

Mean age of the patients was 70.5±6.4 years. Mean follow-up period was 28.2±21.3 months. The mean VA was 0.84±0.42 and 0.37±0.32 logMar (p=0.001), the mean IOP was 22.9±8.2 and 14.8±3.4mmHg (p=0.001) and the mean number of medications was 3.7±0.5 and 1.3±1.4 (p=0.001), preoperatively and at last visit, respectively. The complete success was provided in 14 eyes (44%) and the qualified success was provided in 27 eyes (84%). In 2 eyes additional surgery was performed. In 3 eyes IOP was over 18 mmHg and new glaucoma surgery was planned for two of these.

Conclusions:

In selected patients with advanced glaucoma and cataract, the success rate of combined trabeculectomy and phacoemulsification surgery is high and it can be applied safely. Nevertheless, the patients must be followed-up frequently and if necessary new drugs or new surgeries should be added.
