Trans-scleral fixation of looped haptic foldable acrylic PCIOL in aphakia: a viable solution

Abstract Details
Purpose:
To evaluate the outcome of trans-scleral fixation of looped haptic acrylic posterior chamber intraocular lens (PCIOL) in aphakia.
Setting:
Kaushalya Devi Eye Institute, Pilibhit, India.
Methods:
24 aphakic patients were included. Scleral fixation of looped acrylic PCIOL was performed.
Partial thickness scleral flaps were fashioned 180 degrees apart followed by anterior vitrectomy. The two ends of 10-0 polypropylene suture were tied over the loops of IOL. IOL was loaded in the cartridge and the two ends were trailed out of its nozzle. The IOL was injected into the anterior chamber. The two ends of sutures were taken out through scleral pockets using 26 gauge needle and pushing the IOL behind the iris. The sutures were tied in the scleral bed. Sclera flaps and conjunctiva were closed.
Results:
Out of 29 eyes of 24 patients, 24 (86.2%) eyes had improved postoperative visual acuity. 2(6.9%) patients showed no change in visual acuity. 3 (10.3%) had postoperative complications of which 2 had worsening of visual acuity attributed to secondary glaucoma and macular scarring. Mean best corrected visual acuity (BCVA) pre operatively was 0.78 ±0.39 and post operatively at 3 months was 0.41± 0.52. On applying paired-t test on pre and postoperative BCVA we got a significant p value <0.0001. Pearson’s correlation was used to verify the pairing which also yielded significant value r =0.69; p < 0.0001.
Conclusions:
The use of looped haptic acrylic IOL for scleral fixation appears to be safe and viable alternative to conventional scleral fixated intraocular lenses (SFIOLs). A significant improvement in mean BCVA (p<0.0001) was observed after the procedure.
Use of hydrophilic acrylic showed several unprecedented advantages over the PMMA SFIOL intra-operatively as well as with post-operative outcomes.
