Management of spheroidal degeneration in a hypermature cataract with short axial length and pseudoexfoliation syndrome: a case report

Abstract Details
Purpose:
To report a case management of spheroidal degeneration in a hypermature cataract by Superficial keratectomy followed by small incision cataract surgery.
Setting:
Department of Ophthalmology, Dr. Sulaiman al Habib Medical group, Riyadh, Saudi Arabia.
Methods:
After careful presurgical evaluation and consent 70year old male patient posted for surgery. Intravenous mannitol and oral acetazolamide tablet started 30minutes before surgery to reduce IOP and positive pressure during cataract surgery. Superficial keratectomy completed with smoothening of corneal surface using diamond burr. After placing 4 iris hooks to dilate the pupil, continuous curvilinear capsulorhexis performed. After completing the nucleus delivery through superior sclero-corneal tunnel and cortex aspiration, a +29.5D PMMA IOL implanted in the bag.
Results:
Results are evaluated at 3 levels. 1: Clearing of spheroidal degeneration and smoothening of ocular surface. 2: BCVA improved from Hand movements to 6/18. 3: Management of glaucoma and evaluation of optic disc made easier following the clearences of visual axis.
Conclusions:
Meticulous handling of individual problems in a case of spheroidal degeneration with small pupil, short axial length, pseudoexfoliation syndrom along with hypermature cataract can be managed successfully.
