Comparison of reusable and disposable injection systems of single piece foldable Alcon AcrySof and Rayner IOLs in terms of ease of insertion, time spent and complications during IOL delivery

Purpose:
To evaluate the Alcon AcrySof reusable and Rayner disposable IOL Injection systems in terms of ease of insertion, time spent and complications occurred during IOL loading and implantation of single piece foldable IOLs in the capsular bag.
Setting:
Eye Department Central Park Medical College &WAPDA Teaching Hospital Complex 210 Feroz Pur Road Lahore Pakistan
Methods:
Retrospective study of 379 eyes implanted with Rayner Hydrophilic single piece and 259 eyes with Alcon AcrySof Hydrophobic single piece IOLs from January 2007 to Dec 2014. Routine phaco was performed. Both types of IOLs were implanted using fully disposable device in case of Rayner and disposable cartridge with reusable plunger in AcrySof group. Lens Insertion time (LIT) was calculated from opening of the IOL and its cartridge/injector to the final positioning inside capsular bag. Other criteria used to analyze, included optic haptic catch, overshooting, use of excessive force, stress marks on IOL surface, and difficulty and speed of unfolding.
Results:
All the eyes in both the group had smooth Phaco and intra bag injection of IOLs with their IOL Insertion System. LIT were ±85 seconds in Rayner and ±90seconds in Alcon AcrySof group. NO overshooting was noted in Rayner group because of silicon sleeve. Overshooting of metallic plunger was documented in initial cases with AcrySof group which got settled later on. 6 IOLs got stuck and needed repositioning in the cartridge in Rayner group. One such occurrence was noted with Monarch 2 system. Superior haptic catch/cutting was noted in 10 cases in Rayner group. This was mainly due to misplacement of IOL in the groove.
Conclusions:
A fully disposable system in case of Rayner was a considered a plus point in busy lists. One needs to be careful with Monarch 2 system as metallic plunger may overshoot. Both IOL Injection systems were found to be safe and effective. However it is stressed that proper positioning in the cartridge i.e. extended haptics in case of Rayner and fetal position haptics in case of AcrySof is the key to success in respective groups.
