Purpose:
To analyse IOL implantation behavior and interaction with capsular bag and zonules during implantation of different monofocal, hydrophobic acrylic intraocular lenses (IOL) and to investigate their material properties in a laboratory setup.

Methods:
In this laboratory analysis four IOLs were compared: the Zeiss CT Lucia, the Alcon Acrysof SA60AT, the AMO Sensar and the B&L Envista. We analysed unfolding IOL properties in 15 human autopsy eyes during implantation using the Miyake-Apple-Technique and a high speed camera. Glistenings were created as aqueous-filled microvacuoles in five IOLs per model using an accelerated laboratory method.
Results:
With the high speed camera up to 100.000 frames/sec can be recorded. We used a lower frequency of 5 to 10.000 frames/sec to analyse insertion of the IOLs and the interaction between haptics, capsular bag and zonules during implantation of these IOLs. Due to the hydrophobic nature of the material the IOLs exit the cartridge in the anterior chamber controlled and in a slow manner. The amount of glistenings differed between the examined IOLs. While some IOLs only showed a glistening grade of 0 according to Miyata et al., other IOLs developed glistening grade ≥ 1.
Conclusions:
The four analysed IOL models showed comparable and safe unfolding properties during the implantation process. The analysis of material properties revealed differences in glistenings formation in our laboratory setting.
