Biomaterial analysis of opacified hydrophilic IOLs from the same manufacturer

Abstract Details

Purpose:

The calcification of hydrophilic IOLs is a rare complication. We report about the findings in opacified Euromaxx ALI313Y and ALI313 IOLs (Argonoptics) using light and scanning electron microscopy, x-ray spectroscopy and an optical bench analysis.

David J. Apple International Laboratory for Ocular Pathology, University Eye Clinic Heidelberg, Germany.

Methods:

Five calcified Euromaxx ALI313Y IOLs (Argonoptics) and one ALI313 IOL (Argonoptics) have so far been explanted because patients suffered from a decrease in visual acuity. These IOLs were sent to our laboratory and examined using light and scanning electron microscopy. The composition of the deposits was analyzed by means of x-ray spectroscopy. The optical quality of the IOLs was assessed using an optical bench.

Results:

Macroscopically, the entire optic was opacified. The modulation transfer function curve showed a significant decrease in optical quality. Light and scanning electron microscopy revealed numerous fine, granular, crystalline-like deposits which were distributed in a line parallel to the anterior and posterior surface of the IOLs. X-ray spectroscopy could prove the deposits to consist of calcium and phosphate.

Conclusions:

The calcification of hydrophilic IOLs only occurs rarely. The exchange of the IOL is the only therapeutic option in symptomatic patients. The exact chemical composition of the deposits can be assessed by means of x-ray spectroscopy.
