Risk factors for pseudophakic retinal detachment: the role of posterior capsular rupture with or without vitreous loss

Abstract Details

Purpose:

To investigate the role of different risk factors,particularly posterior capsular rupture with or without vitreous loss- to the development of retinal detachment (RD) after cataract surgery.

Setting:

Single center retrospective cohort, electronic medical record (ERM) study Gloucestershire Hospitals NHS Foundation Trust

Methods:

Data were automatically extracted using audit tools in the EMR on consecutive first eye cataract operations performed between 2005-2014. The EMR mandated collection included: gender, age, axial length, structured list of operative complications [in particular posterior capsular rupture (PCR) with or without vitreous loss], subsequent RD surgery. Age and axial length were stratified into 3 comparably sized groups. A survival analysis was performed with Kaplan-Meier curves and a Cox proportional hazard regression analysis to calculate hazard ratios (HR).

Results:

18,065 first operated eyes were included in the study. The RD rate after 3 months and 7 years was 0.067% and 0.30% respectively. Men had a higher RD risk (HR 2.00; p =0.03) only in the univariate model. Patients < 60 years and those > 80 had a HR of 5.12 (p< 0.001) and 0.16 (p=0.01), respectively. Eyes longer than 25 mm had a HR of 3.98 (p< 0.001). PCR occurred in 400 (2.2%) eyes. The HR for RD was 12.83 (p< 0.001) for PCR with vitreous loss. There were no RD events in eyes with PCR without vitreous loss.

Conclusions:

Younger age, longer axial length and posterior capsular rupture with vitreous loss are all significant risk factors for retinal detachment after cataract surgery. Out data suggests however that gender and posterior capsular rupture without vitreous loss may not constitute a risk for the development of pseudophakic retinal detachment.
