Trans-ciliary fixation of three-piece IOL in eyes with absence of capsular support

Abstract Details
Purpose:
To study the clinical and functional results of implantation of three-piece IOL through clear cornea incision with suture fixation in the ciliary sulcus in eyes without capsular support.
Setting:
Three-piece IOL Secura-s SAY (Human Optics) was implanted in 57 eyes of 57 patients aged from 35 to 85 years. Implantation was performed through clear cornea incision 2,75mm using the injection Monarh system with "C" cartridge (Alcon).
Methods:
IOL haptics vere sutured in ciliary sulcus. Follow-up period ranyed from 12 to 24 months.
The reasons for the initial lack of capsular support were: post-traumatic luxation of the lens into the vitreous cavity – 49 eyes (86%), aphakia after previously performed intracapsular cataract extraction – 3 eyes (5.2%), IOL luxation into the vitreous body – 5 eyes (8.8 %).
Results:
Befor the surgery the mean BCVA was 0,28 ± 0,03. After surgery the best corrected visual acuity 0.4 or higher was observed in 47 cases (82.5%) and average 0,54 ± 0,04. IOL had stable position in all cases in both early and late in the postoperative period.
Conclusions:
Implantation of IOL Secura-sSAY (Human Optics) through a selfsealing corneal incision with suture fixation in the ciliary sulcus in the absence of capsular support allows to achieve good clinical and functional results and IOL stability over a long period of observation.
