Abstract Details

Purpose:

To assess norm values for straylight in the pseudophakic eye as a function of age, and to establish an age-model for straylight improvement following lens extraction. The pseudophakic straylight norm may serve to compare different approaches to cataract surgery and IOL models.

Setting:

Systematic literature review with retrospective data analysis. Rotterdam Ophthalmic Institute, Rotterdam, the Netherlands.

Methods:

A comprehensive review with retrospective analysis of prospectively-collected cross-center data were performed to assess straylight in normal pseudophakic eyes. Studies reporting straylight values obtained with the natural pupil using the C-Quant device (Oculus Optikgeräte GmbH, Germany) after uneventful phacoemulsification and following intraocular lens implantation were included. A literature review identified 16 papers on straylight and pseudophakia with no comorbidities. The postoperative results were used to define the norm for straylight in pseudophakia. The straylight improvement following lens replacement was assessed by evaluation of pre- and postoperative values. An age-effect was incorporated to determine a model for straylight improvement.

Results:

The mean (± standard deviation) postoperative straylight value, derived from 16 studies (1869 eyes), was 1.21 (± 0.21) log units. Age dependence could be assessed from 13 studies (1533 eyes) resulting in the straylight-age norm curve in pseudophakic eyes. A strong correlation was observed between preoperative straylight and its improvement after cataract extraction with a clear dependency on age.

Conclusions:

We developed a norm for straylight in the pseudophakic eye. This norm is considerably different from the previously published one for the phakic eye. The new pseudophakic norm can be used clinically for predicting straylight improvement (or deterioration) after lens replacement, and as a reference criterion for clinical studies.
